Paper Based Medical Record Vs Personal Health Records System The Various Forms of Personal Health Records Personal Health Records (PHRs) may be created and maintained in varied formats such as paper-based and electronic, and allow control of different degree to access as given by individual or provider. Differences in access may be caused by permission needs and such legal factors, reliability of electronic devices, Internet connectivity, and type of the organization. Major Classification of Health Information Systems Electronic Medical Records (EMRs): These are the digital versions of the erstwhile paper chart-based documentation of health information in medical practices and clinics. EMRs consist of clinicians’ notes and health data that are used by practices regarding diagnosis and treatment. Personal Health Records Software

EMRs prove to be far better than their paper-based counterparts because of their capability to enable practices track health data over time, monitor patients, identify individuals for specific screenings and visits, and generally increase quality of health care. Electronic Health Records (EHRs): Going a step beyond standard health data, EHRs are developed to include a broader perspective about patients’ health care. They are supposed to include health information of the patients collected from all the medical facilities and physicians at which they have received medical services. EHRs allow sharing of health information of patients among different medical care providers like specialists and laboratories. They store and share patient information gathered and managed across different geographical locations. Personal health Records (PHRs): While consisting of the same type of health data as found in EHRs, PHRs have been designed to be created, shared, and edited by patients themselves. Although the information contained in PHRs are similar to that in EHRs, namely, medical history and medications, allergies and immunizations, and contact information of patients and their providers, PHRs are being managed and controlled by the patients. PHRs offer secure, private, and confidential environment; they are maintained mostly online using the Internet, and the information included are drawn from various sources such as clinics, physicians’ devices, and the individuals themselves. Difference between Paper-based and Electronic Health information Management In the past, similar to any other record, medical records were maintained in paper format. This involved using a lot of space and number of human resources for effective upkeep. Even after tedious processes and voluminous staff, efficiency was not on par with requirement.
