Demographics and Lifestyles Make a Difference • Medical cost increases continue to outpace inflation • Alcon’s aging workforce and low turnover rate mean even greater increases in medical costs and more lost productivity over time (obesity and high risk factors) • Unhealthy lifestyles lead to more health care risks and increased medical costs

Alcon Health Plan - New Vision • Goal #1 - Provide health promotion resources that assist plan participants in: • containing health risk escalation for healthy individuals • identifying health risks and improving health status for individuals “at risk”. • Goal #2 - If ill-health presents itself, provide access to high quality medical care at the appropriate level and in the most cost effective manner.

A New Approach to Healthcare:Moving from Illness to Wellness • Focus is on prevention and wellness, rather than just treatment of illness • Provides programs and tools to reduce specific health risks and encourage healthy lifestyles • Encourages employees and dependents to use premier UHC network providers as well as utilization management programs (future incentives are being considered).

Timeline • Health Risk Assessment implemented during 2005 ( August - October) • Premier Medical Plans launched January, 2006 • $200 contribution to Health Reimbursement Account made in January, 2006 • Health Assessment offered again in 2006 (August – September) • Employee and covered spouse eligible for another $200 HRA contribution in January, 2007 based on participation in “health coaching” or physical activity program.

  18. The Health Assessment Questionnaire • Gateway to: • 2 new “Premier” medical plans with enhanced benefits • Supported by free lab screenings to measure cholesterol and blood sugar for employees and covered spouses.

Blood Screening Test Biometric screening values: cholesterol (HDL/LDL), blood sugar, blood pressure, height and weight Two options were provided (self reported on HA): • Recent Results: • Tests performed by personal physician if tests occurred in the last 6 months, or • Visit to a local lab: • Special lab requisition form and UHC medical ID card, • Website to find locations – national sites

Health Reimbursement Account Contribution Employee has two ways to qualify for the $200 company contribution for 2007: • Track physical activity on MediFit’s Virtual Fitness Center website – at least 20 minutes/day, three times per week for 10 weeks out of a 12-week period. • Complete telephone-based Staywell Health Coaching sessions • Both employee and covered spouse must qualify

How Does A Participant’s Health Condition Affect Eligibility For Benefits? • Choice of health plan or employee rates are NOT affected by any health condition, screening test results or the information entered into the Health Assessment questionnaire. This information is not shared with Alcon. • Eligibility for Premier plans is based only on whether the Health Assessment is submitted by deadline.

  23. Privacy Is Protected! Confidentiality of Health Information is Critical to Program Success: • The health information provided in the Health Assessment is protected by StayWell in a completely confidential manner. • Individual health data will not be shared with Alcon in accordance with privacy laws. • Staywell Wellness Manager coordinates invitations to special on-site risk reduction programs. • Carefully avoided “back door” release – for example, nonparticipation data.

Deadlines • Blood Screening: July – Sept Provided guidance that two weeks are needed to receive screening results. • Health Assessment Submittal: August 15 - September 30 • Annual Open Enrollment (PPO or EPO): November • New Plan effective January 1, 2007 • Physical Activity Tracking/ Health Coaching Deadline: October 31
