  3. Please use this form for all evidence of insurance on Master Condo Associations. Please note that no other proof of insurance questionnaires or loan tools will be completed by ThompsonBaker Agency.

  4. Certificates of Insurance or Flood Declaration Pages are the only documents that will be provided as proof of coverage for our Master Condo Association policies.

  5. Certificates are processed in 24 hours, in the order in which they are received.
