Choctaw Nation Youth Empowerment program • Working within the communities and schools to promote healthy living and well being. • Empower youth by helping them discover their role as a leader, build self esteem and guide them to furthering their education. • Project Falvmmichi- a domestic violence prevention program using teen mentors to teach second grade. • Youth Advisory Board- 8th-12th grade that volunteer in their communities, educating and promoting among their peers and underclassmen subjects covering hygiene, self-esteem, bullying, nutrition, fitness and substance abuse.

JonesAcademyLearnand Serve Project • Empower the youth • Educate our youth on prevention methods and alternatives to substance abuse among the student population. • Emphasize the importance of service to the community and service-learning activities as a substitute to the use of methamphetamine and/or other drugs.

Reach U.S. program • Promoting employee and community programs and events, as well as policy change which focuses on physical activity, nutrition, and tobacco prevention. • Goal: Reduce or eliminate health disparities among American Indians • Annual Events Sponsored by REACH • 5K Runs • Softball Tournaments • Youth Fitness Camps • Wellness Team provides • Education to 3rd grade students & CN employees • Fitness assessment and reporting program • Tobacco education cessation classes

Choctaw Nation Healthy Kids Program Working with Families to Support Healthy Lifestyles! • Program participants identified by CN pediatricians • Ages 11-14 • Weights 180lbs-418lbs • B/P 119/77-135/82 • BMI 33.57-59.98 • A1C’s 5.4-6.0 • Learning together about healthy lifestyles • Dietician, Raina Sparks • Activity Coach, Amber Blue • Parent Support/Coordinator, Lee Ann Griffin • Healthy Lifestyle Motivator, Courtney Trent • Physician, Dr. Robinson • Once a month office visit at DWC for 5 months • September 15th, October 20th, November 17th, December 15th, January 19th • Frequent phone contact with your DWC support staff during the week • Activity logs • Parental support Overall Goals • Reduce Weight, A1C, & BMI. • Prevent diabetes among our Native children by giving families the tools & support to live healthier lifestyles!

Jones Academy Prevention ProgramRaina Sparks MS, RD/LD, Registered Dietitian • Students are followed throughout the year: • Pre-test at the beginning of the school year and a post-test at the end of the school year. • Data is collected three times a year: Ht, Wt, BMI, Blood sugars, and Blood Pressure • The registered dietitian works very closely with the kitchen staff on preparing healthy meals and snacks. • Students learn life skills to prevent diabetes, heart disease and obesity. • We want these children to grow to be healthy adults who can teach others the importance of living a healthy lifestyle.

Choctaw Nation Wind HorseFamily Recovery Center • Strengthening Families • 14-session, science-based parenting skills, children's life skills, and family life skills training program • Outcomes • 14-session, science-based parenting skills, children's life skills, and family life skills training program specifically designed for high-risk families. Decreased use and intention to use alcohol, tobacco and other drugs. • Enhanced children's protective and resilience factors by improving children's social and life skills, peer resistance, and communication skills. • Improved parent/child attachment and family relations, communication, and organization.

School Based PreventionPositive Action • Educating Youth to live Healthier Lifestyles, Physically, Emotionally, & Mentally! • Implementation of “Positive Action” life skills and substance abuse prevention education for elementary children in Pittsburg and Latimer Counties (estimated impact on 18,000 school children during course of the program. • Media messages to promote substance abuse prevention through radio, television, and print information.

Healthy Lifestyle Coach Courtney Trent, MSW • Courtney is currently the Wellness Counselor at DWC, she utilizes techniques such as Motivational Interviewing to assist patients in identifying barriers that may be keeping them from making good health choices and identifying solutions that will lead to a healthier lifestyle. • Courtney’s role as the healthy lifestyle coach • Assist pt’s in identifying barriers • Motivation • Family/pt Support • Topics for Healthy Kids: • Living healthy, physically, mentally, and emotionally • The Eagles Nest (Enthusiasm, Energy, Listen & Learn, Good ways, & Activities • Managing diabetes at school; healthy routine What’s Yours?
