  2. Patient: Mrs G Khanna • Age: 45 years old • PMH: Arthritis (Celebrex) • Chief Complaints: • 1. Unhappy with colour of upper front tooth. • 2. Would like something done about lower front teeth.

  3. PDH: • Tooth 21 underwent RCT 18mths ago after an accident. • Missing tooth 31, lower anterior bridge made >20 years ago, most unhappy with the discolouration and display of metal.

  4. Provisional Tx Plan: Tooth 21 Internal bleaching Veneer/Crown Tooth 41 and missing 31 Maryland Bridge Zirconia cantilever bridge Crown 41, single tooth implant for 31
