ACCIDENTS GROUP CLASSIFICATION • Loss of control following lightning strike and/or total loss of electrical power- Group I • Loss of control following an instrument failure or malfunction (attitude/air speed indicator)- Group II • Loss of control due to cargo loading errors- Group III • Loss of control following air intake icing on turboprop aircraft engines- Group IV • Loss of control due to spatial disorientation (resulting in spiral dives)- Group V • Loss of control due to the use in-flight of reverse beta range- Group VI LoC Ad Hoc WG

  3. LoC following Lightning Strike and /orTotal loss of Electrical Power • Aircraft airworthiness protection against lightening strike EASA Interface (Discuss with Rulemaking Directorate for considerations in CS-23). • Pilot training for recovering from total loss of electrical power  Action for the FCSG to look and study this issue. LoC Ad Hoc WG

  4. LoC following an instrument failure or malfunction • AFM procedures including unreliable airspeed indications  Action to be coordinate with EASA Flight Standards Manager, Even Nielsen. • Pilot training for scenarios in which airspeed indications are unreliable Action for the FCSG to study this issue. • Is it possible to simulate unreliable airspeed indications?  Action for the STD SG to study this issue. • Need to install an additional standby attitude indicator (artificial horizon) for aircraft with MTOM less than 5,700 kg?  Action for the EQSG to study this issue.   5. LoC due to cargo loading errors • Quality Audit on Loading Instructions given to flight dispatchers  Action to be studied by SWSG. • Is JAR-OPS 1.205 ‘Competences of Operations Personnel’ robust enough to prevent those loading errors?  Action for the SWSG to study. LoC Ad Hoc WG

  6. LoC following air intake icing on turboprop engines aircraft • AMM recommendation for parking in icing conditions? Action to be coordinated with EASA Maintenance Experts and Aircraft Manufactures. • The ‘auto-relight’ modification. Human factors’ aspect? Action for the FCSG. • SIC (CJAA Operations Division Safety Information Communications) for operators to recommend the NOTAC issued to aircrew to inspect the air intakes(coordination with EASA needed)  Action for Icing SG LoC Ad Hoc WG

  7. LoC due to Spatial Disorientation • Training awareness programme focussed on pilots with an Eastern Europe/Russian training background Action for the FCSG. • Installation of an aural ‘bank angle’ warning.  Advice and suggestion from Boeing and Airbus and other A/C Manufactures. LoC Ad Hoc WG

  8. LoC due to the use in-flight of reverse beta range • To issue a recommendation for an OD (Operational Directive) to highlight to the operators that it is not permitted to use the beta-range in-flight on turboprop aircraft. • If included in AFM (EASA Flight Standards) => SIC (JAAOPSDIV Safety Information Communications) • If not included in AFM => Operational Directive avoiding the use • New aircraft should be fitted with a device to prevent the use of beta range in-flight EASA Rulemaking Directorate, Yves Morier confirmed so. Need for retrofitting existing A/C? A RIA should be developed LoC Ad Hoc WG

  9. ADDITIONAL ACTIONS • Possible lack of training and/or awareness of the implications of allowing approach speed to fall below Vmcl on turbo-prop aircraft (especially during single-engine approaches, for example with an engine out).  Action for FCSG • Pilot Awareness of one-engine out, go-around from landing configuration case? Need for training on this configuration. FCSG LoC Ad Hoc WG
