Training Objectives • The main agency recommending blood glucose training and explain the importance of training • Identify contraindications, know where to find this information and explain their importance • State when you have to quality control your meter and explain why you have to do this • Identify the correct procedure for doing a test and demonstrate your knowledge by performing a QC/Self Test • Identify your point of contact within the hospital and explain some basic troubleshooting steps • Where you get your meter supplies?

Why Training? • Medicine and Healthcare Products Regulation Agency (MHRA) • Nursing and Midwifery Council. • Royal College of Nursing • Department of Health • Local Trust “The use of blood glucose meters by untrained staff without adequate management supervision of the equipment and without the use of quality control procedures, can lead to misleading results, adversely affecting the treatment of patients.”

  4. Contraindications for use of Blood Glucose Meter Due to the Hazard Notice the MHRA recommend that therapeutic decisions should notbe based on a capillary sample in these circumstances: • Dehydration • Hypotension • Shock • Peripheral Occlusive Disease • Hyperosmolar Non-Ketotic Coma (HONK) • Decompensated Heart Failure NYHA Class IV • Diabetic Keto-Acidosis • Unconscious Patients Venous Sample should be sent to the Lab for verification of blood glucose level

  5. LIMITATIONS. • Due to medical conditions: • Galactose > 1.11 mmol/L (overestimation) • Lipids > 16.8 mmol/L (overestimation) • Sodium > 150 mmol/L (underestimation) • (Hypernatremia) • For further information refer to Test Strip Information Leaflet, • Workstation insert or Ward Manual

  11. Quality Control Testing WHY? WHEN? HOW? • Internal Qc testing – • Refer to ~Hospital/Community Protocol – • Qc Daily before Patient Use / Qc weekly & on day of use for patients. AND ~ New Test Strips, New battery, ~ If meter replaced ~ Patient picture not reflective of result. • Documentation of Results & Accountability • EQA – Monthly Sample sent out from Laboratory

Failed Quality Control Test • Check expiry dates, mix QC solutions and repeat test • Change quality control solutions (write new 3 month expiry date) • Change test strips (remembering to change code chip) • Document all findings in QC log book • Contact relevant department in hospital report fault and obtain replacement meter If meter continues to fail DO NOT use it!

  16. Verification of Blood Sample • Always perform a visual inspection of test area. • If yellow area is visible – Perform a repeat test ensuring yellow area fully covered.

Important Error Codes Meter Reading Range 0.6mmol/l – 33.3mmol/l Send a venous sample for verification if reading < 3mmol/l or > 20mmol/l or according to your Trust Policy/Guidelines for Blood Glucose Monitoring. Please refer to Ward Manual/Logbook for all display/error codes. Error codes which mustbe acted upon immediately are shown below, these codes indicate the result is potentially outside the meter reading range: Potentially very low glucose Potentially very high glucose

  18. The temperature is above or below the proper range - i.e 6 – 44oC. Meter requires to be warmed & takes 10mins before usable. The code chip is from expired lot of test strips. Insert new in date code chip. ERROR CODES: (see back of log book)

  19. Cleaning Meter • Damp cloth & detergent • Alcohol wipes 70% or less may be used. • Follow hospital policy for decontamination
