  2. ICF/ID Transition Program • What is it? • The Transition Program is a way for people living in a facility (like the one you live in now) to move to another place (like a group home or other community setting).

  3. Program Participation • The Program is voluntary. You don’t have to participate if you don’t want to. It is your choice. • If you don’t participate this year, it won’t stop you from choosing to participate in future years.

  4. Program Participation • Will everyone who is interested in the Transition Program get to move from an ICF to a group home or other community setting? • Probably not. There is a limited amount of money for the Transition Program (enough money for about 48 people to participate this year). If more people are interested in the program than funding allows, not all of them will be able to participate.

  5. Program Education • The State will send letters to potentially eligible people and their families to let them know: • Transition Program enrollment is happening • Education sessions will be held in person at the place where you live • Education sessions will also be held by conference call • After the education sessions have been completed, the State will contact people and their guardians to see if they are interested in participating.

  6. Selection to Participate How are individuals selected for the Program? • The State makes a list of all the people living in facilities who have both Utah Medicaid and have lived in a facility for at least 12 months in a row. • Then the State makes two lists: • A list that puts people who have shown interest in moving into the community (by being on the DSPD waiting list or expressing interest in participating in the ICF Transition Program in a prior year) and lived in a facility for the longest amount of time at the top; and • Another list with names in random order. • Then the State starts at the top of both lists and contacts people until it finds about 48 people who want to participate.

  7. Program Participation • If I am selected for the Transition Program and choose to participate, what will happen next? • You will have help to decide where you are going to live and who you want to help you with the services you get. A person from Utah Medicaid or the Division of Services for People with Disabilities (DSPD) will meet with you and your family, or others you invite, to help you decide. • You will move out of the facility you live in now and will move into a group home or other community setting.

Program Participation • If I am selected for the Transition Program and choose to participate, will I get help with the same kinds of things I get help with now? • Yes, the table below shows the kinds of help you will get with things you need – whether you stay in the facility you live in now or if you move to a group home

Program Participation • Will someone be available to help me anytime, day or night, if I need it? • Yes, someone will always be available to help you if you need it. • What are some things that may change if I am selected to participate in the Transition Program? • If you move from where you live now to a group home or other community setting there will be some things that will work a little differently. The table on the next page shows you some things to consider.
