Patient on two or more maximum tolerated oral hypoglycaemic agents with raised Hba1c of > 7.5% 2. Patient already on insulin having problems with: ? Hypoglyceamia ? Review or change in type of insulin or regime ? HbA1c raised >7.5% despite insulin titration Exclusion Criteria: ? Already under the Hospital Diabetes Team - contact hospital DSN - North Mid Hospital - 0208 887 4238/4272 - Whittington Hospital - 0207 288 3344/5767 ? Home visits limited service Clinics: ? Lordship Lane HC ? Laurels HLC ? Bounds Green HC ? Crouch End HC/ Hornsey Central HC Refer Using SAP Referral Form : Add Refer to Diabetes Specialist Nurse under Reason for Referral with explanation
