Objectives for this Powerpoint • Provide a way for you to organize your paperwork for 4th year (you can also set up electronic files using this design) • Provide a way for you to cut down on the time it takes to apply for away electives • Provide a place for you to store your important information

  3. What’s inside the Binder? • For the remainder of this PowerPoint, consider each slide to be a pocket in a binder (or a folder on your computer) that contains important information for the 4th year. • The title of the slide is the name of the folder. • The bullets are what you should put in each folder.

  4. Inside pocket of binder cover • Academic calendar that shows July 1st through June 30th of your 4th year • Use a calendar that has a box for each day so you can write in it if you choose to.

Folder # 1: 4th yr schedule & Internship Quest Time Table • 4th year schedule printed from Cbase • Blank calendar page from Cbase2 registration • Print the webpage: http://sbumc.informatics.sunysb.edu/som/InternshipQuest • From the above webpage, print the link under Figure 1 called GENERAL RESIDENCY APPLICATION TIMELINE (figure1)

Folder # 2: Match & ERAS (Include SF Match or AUA Match info if you use those Match programs) • Request for Letter of Recommendation/Cover Sheet (print from MyERAS or pick up in OME) • Create a list of Match sites and include your AAMC ID # on the page(e.g. http://www.auanet.org (http://www.auanet.org/content/residency/residency-match.cfm), http://nrmp.org, http://www.aamc.org, http://sfmatch.org) • The Main Match schedule (http://www.nrmp.org/res_match/yearly.html) • ERAS information you might need (http://www.aamc.org/students/eras/start.htm) • SF Match Timetable: http://www.sfmatch.org/general/general_timetable.htm • Print and keep emails we send you about the Match and about ERAS

  7. Folder #3: List of Useful URL’s • List of Match sites • Cbase URL • Cbase2 URL (in case you want to drop/add or swap) • URL’s of residency sites that interest you • You could also include important phone numbers if you haven’t put them in your cell phone yet.

  8. Folder #4: CV, MCAS, Other Personal Information and Documents • Personal Statement—3 or 4 copies • CV—3 or 4 copies • MCAS report for personal information to keep on hand • Your copy of your USMLE reports of Step 1 results — 3 or 4 copies • Certificate of completion for Infection Control/OSHA course

Folder # 5: Health/Insurance Information • Health insurance card-5-6 copies • Immunization forms (with records of vaccines, flu shots, updated PPD’s, annual physical assessment)—5-6 copies • A blank Health Update form (pick up from Student Affairs office-Diane) • Copies of any health forms required by away sites where you will apply for electives

  10. Folder # 6: Residency Information • Record of your interviews (Give each site a rating according to your priorities. You might list, for example, housing, transportation, friendly environment, location, etc.) • Print outs of residency info –e.g. schedule overview for the department you apply to • Pictures of residency sites • Interview process schedules • Application procedure and housing • Other pertinent information regarding interviews and sites

Folder # 7:USMLE • URL of helpful web pages including study guides, question banks (perhaps a Kaplan site or NBME websites) • Copy of USMLE application printed out • Copy of Prometrics information (URL, phone no.) • Copy of ID you plan to use when you go to take the exam • URLs for Prometric dates when exams are available • 4-5 copies of your Step 1 results

Folder # 8: Elective Forms • Elective approval forms for off campus and international electives • ITP (International Travel Policy paperwork) • Drop/add form and URL for “how to set up electives” website (https://cbase.som.sunysb.edu/cbase2/public/course_info/electives_info.cfm) • Elective Evaluation form • Elective grading sheet • Research Elective forms • VSAS URL, password, instructions Thank you to Shannon Fuller for these innovative ideas!
