  3. Are Identified Probable Users Getting FP Services? Service provision not keeping up with identified probable users • Unserved probable users in current month are added to succeeding month. • Acceptors are also cumulative

Reporting Protocol RHM provides overall supervision to CHT Partners and NDP, provides services including counseling, records in the facility and home-based records, provides appropriate health messages and uses the logbook to find out who else needs services and should be followed-up by the CHT Partner CHT Partner uses the Logbook in profiling, master listing, giving of messages and accomplishing of Family Health Record Form • NDPchecks the entries in the LOGBOOK and helps the CHT Partner in: • Correctly filling-out the logbook, Family Health Record Form and Masterlist • Polishing the messages
