  2. Aim By 2010, all disabled young people in Birmingham should have a person centred transition plan using the Transition Pathway

  3. Transition Pathway Implementation • Multi Agency funded training • Wide range of participants – schools, health and social care workers, Connexions, 3rd Sector agencies • 59 schools trained to date including all special schools • Targeted approach for mainstream schools based on % of statemented pupils

  4. Transition Pathway Implementation • SENAS accept Transition Pathway Plans and documentation • 200+ plans submitted to SENAS • Links with CAF for monitoring and analysis • Follow up to training – now includes briefing on post 16 and post 19 training opportunities • IAG – leaflets and events

  5. Independent evaluation with Wave 1 Support and commitment from senior managers Named person with responsibility, authority and time to oversee implementation Action Plan has been developed and implemented TP has been incorporated into the curriculum – often via PHSE and citizenship Feedback and Evaluation

  6. Development Areas • Monitoring • Quality Assurance • Needs identification • A Pathway to where? • Resources and time • Partnerships and links with other PCP tools and assessments • Future training and support issues

  7. 2009/10 Plans • Focus on implementation with existing schools • Targeted approach to training including new SENCOs • Links with SENAS and CAF • Marketing via the City Council website • Further evaluation
