    4. 4 Purpose To record faculty activities in the following areas in a convenient and accessible manner that will allow up-to-date reports at individual, divisional, departmental, and SOM-wide levels. Education – teaching hours, teaching in clinical settings, directing research with students, etc. Research - Publications and Grants/Contracts (reported in $ and faculty effort) Clinical - New Mexico Medical Group & Cancer Research & Treatment Center (CRTC) activity reported as work RVU’s (Relative Value Units) Administration - administrative positions & committee participation

    5. 5 Goals History To gather as much data as possible from central sources Ensure data are accurate. Formerly known as Faculty Profile Before May 1999 faculty entered data on the internet Now - central data sources and data entry by department staff on the internet website.

    10. 10 FAD Website Authorizations Data Entry – Department Staff Information entered by department staff or uploaded from central sources appear in the individual faculty report. Department Reports – Department Chairs & Administrators Individual faculty productivity is summarized by division and by department totals in department management reports at the Department Report Website.

    12. 12 Moving around the department entry website: The website is divided into frames Top Time Period & Website Name (on the left) Website Icon (on the right) Left hand frame -Menu Welcome (starting page) Search (type in all or part of faculty name) Faculty (click on to display faculty) Periods (change time period)

    17. 17 Edit Publications & Copy to Click on edit and the entry opens up to the data entry screen Save again after making changes Click on edit and scroll down to the bottom of the data entry screen. Names of all department faculty will be listed If two faculty in the same department collaborated on a journal article, click on the second author and the publication is copied into the second author’s publication list Note when copying a publication, you are now in the second author’s record. To get back to the original faculty member, click the name in the left hand frame

    18. 18 Committees Committee data entry is optional (check with your department administrator) Enter type of organization, name of committee and faculty member’s role on the committee Save after entry To Correct a mistake delete and start over Each committee has an end date of “continuing” until a termination date is entered When a faculty member is no longer a member of a committee, open the time period when the appointment ends and pick a termination date.

    20. 20 Positions Administrative Titles listed on faculty contracts automatically loaded into FAD Positions (titles) not included in a faculty contract may be added to FAD by department staff Rank is automatically loaded into FAD so do not enter rank in the “Position” entry Rank = Professor, Associate Professor, etc.

    21. 21 Example below illustrates the position of Executive Medical Director which is uploaded from the faculty contract. The added position of “Internal Medicine Residency Director” was added by department staff. Only those positions added by staff can be deleted.

    23. 23 Education Tracking Guidelines In 2003 the Education tracking form was revised to accommodate department specific teaching and activities. There are 2 sources: Central Sources (see Database Source Chart) and Department entry. Sections A to G exist for every department, however, the individual items within each section will vary from one department to another.

    27. 27 Education Data Entry – Click on Education, enter numbers, click on “save” button bottom left

    28. 28 Grant Note Entries in FAD Only Department Administrators may add a “Grant Note” to explain participation in Program Project or Center Grants. These grants may not be represented in the grant information automatically loaded from the Pre-Award Database. Especially if the principle investigator is in another department.     29. 29 Honors Purpose:  To create an information record of SOM faculty accomplishments and their success in receiving awards What:  Professional honors for specific situations or life time achievement awards and/or honors. Professional Organization Officers or Honorary Government Positions. Information collected: Name of honor, organization, monetary award, description of honor

    30. 30 Enter Honor, click “save” Edit by deleting and starting over

    31. 31 Faculty Report Click the “Report” button next to honors. The faculty report has four separate areas 1) Education 2) Research 3) Administration 4) Clinical Everything you entered will show up on the report if it was saved.
