Placentia-Yorba Linda Unified School District By law, all students entering Grade 7 will need proof of the whooping cough booster shot (Tdap) (given on or after their 7th birthday) before the first day of school for the 2014-15 school year. You are encouraged to bring in proof of your child’s Tdap booster shot to your current school before the end of this school year. Proof of Tdap immunization will also be accepted at 7th Grade registration. Avoid the back-to-school rush and make an appointment with your child’s doctor as soon as possible. for parents Assembly Bill 2109 REQUIRES parents who elect to sign a personal beliefs exemption form to meet with a health care provider and sign the Personal Beliefs Exemption to Required Immunizations form in their presence.

Distrito Escolar Unificado Placentia-Yorba Linda Por ley, todos los estudiantes que van a iniciar 7mo grado necesitarán prueba del refuerzo de la vacuna contra la Tos-ferina (Tdap) (administrada en o después del cumpleaños número 7); y necesitará adquirirla antes del primer día de clases del año escolar 2014-15. Le pedimos que lleve a su actual escuela, el comprobante del refuerzo de la vacuna administrada a su hijo antes de que termine este año escolar. La prueba de la vacuna Tdap, también será aceptada al inscribirse en 7mo Grado. Evite las prisas del regreso a la escuela y haga una cita con el médico de su hijo lo antes posible. for parents El Proyecto de Ley 2109 REQUIERE que los padres de familia que firmen una exención de acuerdo a creencias personales, se reúnan con su proveedor de salud y firmen la forma de Exención de Vacunación Requerida por Creencias Personales, en su presencia.
