HISTORÍA En el proceso de programación de valoración por Fonoaudiología el 30% de las consultas presentan error en la programación, debido a fallas en la verificación del convenio o en la inasistencia de los usuarios particulares. De 200 consultas 60 presentan fallas de programación. Actualmente se está en proceso de mejoramiento.

  5. DESCRIPCIÓN DEL PROYECTO

  6. FALLAS ENCONTRADAS-VARIABLES CRÍTICAS ENCONTRADAS • De asignación: de acuerdo al motivo de consulta se asigna de manera errónea la cita. • De verificación: no se verifica el convenio institucional previo a la cita.

  7. ACCIONES • Formar a la persona asignada en el proceso de programación de cita para la selección del área de consulta, utilizar un listado de chequeo de motivos de consulta frecuente como herramienta facilitadora. • Evaluar mensualmente los controles usados para verificación de convenios interinstitucionales e implementar otros si es necesario-Listado de instituciones, solicitud de carnet o carta del Colegio.

  9. LISTADO DE INSTITUCIONES EDUCATIVAS QUE TIENEN CONVENIO • Colegio Leonístico • Institución Educativa Celmira Bueno • Colegio Miguel Ángel Buonarrotti • Liceo Los Cisnes • Colegio Colombia • Colegio Ángeles de Dios • 37 Hogares infantiles que tienen convenio con Club de Leones.

  10. EVALUACIÓN CONSTANTE A partir de la verificación de los motivos de consulta y de la pertenencia a una Institución Educativa que tiene convenio se busca incrementar la eficacia de proceso de programación de valoración Fonoaudiológica.
