CK19 Cytokeratin 19 (CK19) also known as keratin-19, is encoded by the KRT19 gene and the protein molecular weight is about 40 kDa. CK19 is one of the members of the keratin family. The keratins are divided into cytokeratins and hair keratins. As intermediate filament proteins, the CKs (cytokeratins) contribute to the structural integrity of epithelium, cell signaling, stress response and apoptosis. There are more than 20 CK members and consisted of two subtypes: acidic type I (CK9-CK20) and basic type II (CK1-CK8) keratins. These two different keratins form pairs of heterogeneous protein to function. CK-19 is a type I keratin and is the smallest cytokeratin expressed in simple or stratified epithelium and in many tumor cells such as breast cancer cells. The Detection of CK19 There are several approaches have established to determine the CK19 protein level and mRNA level in carcinomas. Recently, emerging studies tried to detect the CK19 mRNA level by RT-PCR of the tumor cells which transfer to the blood, bone marrow and lymph node in breast cancer patients. Another approach to elevating the expression of CK19 in cancer patients is the detection of CYFRA 21-1 by ELISA, immune cytochemistry, FACS, etc. This is because CYFRA 21- 1 is a soluble fragment of cytokeratin 19 that released into to blood. The mechanism of how the soluble fragment is released into the blood is a proteolytic cleavage by caspase 3. The transformation of epithelial into malignant cells increases the keratins, and during the necrosis of the malignant cells, CYFRA21-1 is released into the blood.

CK19 Marker of Breast Cancer As a cytokeratin protein, CK19 mRNA is increased in many epithelial tumors including breast cancer, papillary thyroid carcinoma, lung cancer, and colorectal carcinoma. It has been indicated that there is a close relationship between the blood positive CK19 and distant metastatic breast cancer. Also, efforts have been taken to detect the pre-surgery CK mRNA levels of the peripheral blood of patients with breast cancer to investigate the role of CK19 in the prediction of non-sentinel lymph node metastasis. In a word, CK19 is a specific epithelial diagnostic marker can be used to monitor the progression and predict the response to chemotherapy of breast cancer.
