  2. 台灣LASIK手術的沿革 • 1.1996年： 正式引進台灣，仍屬管制儀器，只有區域 以上教學醫院才能購買。 • 2.1999年： (1)眼科醫學會成立「角膜屈光手術小組」 與衛生署溝通，於7月19日公告「準分 子雷射…即日起不屬…昂貴或具危險性 之醫療儀器項目」眼科診所可購買。 (2)雷射儀器由20多台增加至30台左右。 德照眼科

  3. 3. 2000年： LASIK手術篷蓬勃發展，劇增為80多台。 • 4. 2002年12月： 眼科醫學會設立「雷射屈光手術委員會」。 • 5. 2003年： 準分子雷射儀增加到150台左右 引進飛點小光斑及前導波 施行LASIK手術的眼科醫師已達300人以上。 • 6. 2004年： 眼科醫學會將主導「雷射屈光手術醫師及醫療院 所認證事宜」，以導正坊間雷射屈手術之不當廣 告，渲染，促銷等違反醫學倫理的不良行為。 德照眼科

  6. 名稱 • 台灣白內障及屈光手術醫學會 • 英文全稱為Taiwanese Society of Cataract and Refractive Surgeons（TSCRS）

  7. 宗旨 本會為依法設立，非以營利為目的之社會團體。 宗旨如下： • 一、提升眼科白內障暨屈光手術之醫療水準。 • 二、推廣教育訓練與研究。 • 三、促進國內外白內障暨屈光手術領域的學術交流。 • 四、加強與政府及私人有關機構的聯繫與溝通。

  8. 任務 • 一、發展我國白內障暨屈光手術之研究。 • 二、推動與國內外有關團體機構及個人之聯繫與交流。 • 三、舉辦有關眼科醫師之教育及訓練。 • 四、加強與政府或私人有關機構之溝通與協商。 • 五、維護會員之權益及醫療業務之安全。 • 六、促進會員之和睦和互助，改善醫病關係。 • 七、出版有關眼科刊物。 • 八、其他有關本會宗旨之工作。

  18. 雷射屈光手術醫療院所- 眼科醫學會認證準則 1. 作業：申請 → 書面審查（小組）→ 實地審查（小組） → 複審（理事會）→ 合格證書 2. 規定事項： （1）有效期限：6個月 （2）認證費用：NT$1,200元（含證書） （3）醫師及醫療院所對外公佈資料（限於院所100公尺） 醫師姓名，地址，電話 LASIK, LASEK, PRK 準分子雷射屈光手術 準分子雷射中心 （4）除以上資料外，不得公佈其他有關雷射屈光手術之資料 （5）認證者，公告於學會網站 德照眼科

  19. 雷射屈光手術醫師- 眼科醫學會認證準則 1. 衛生署核發眼科專科醫師證書。 2. 作業：申請 → 書面審查（小組）→複審(理事會) → 合格證書 3. 規定事項： （1）有效期限：壹年 （2）認證費用：NT$500元（含證書） （3）醫師及醫療院所對外公佈資料（限於院所100公尺內） 醫師姓名，地址，電話 LASIK, LASEK, PRK 準分子雷射屈光手術 準分子雷射中心 （4）除以上資料外，不得公佈其他有關雷射屈光手術之資料 （5）認證者，公告於學會網站 德照眼科

OMIC • OPHTHALMIC MUTUAL INSURANCE COMPANY • (A Risk Retention Group) • SUPPLEMENTAL QUESTIONNAIRE – PRK, LASIK and other accepted variations • (including LASEK, IntraLASIK, Custom-Contoured Ablation, etc) • OMIC requires special underwriting review of physicians requesting coverage for the performance of refractive surgical procedures. Coverage is not included under the policy until and unless approved and specifically endorsed. • TRAINING AND EXPERIENCE • 1. The laser manufacturer may require that LASIK surgeons complete a separate certification course in PRK to become certified on the laser. Separate training/certification is required for Custom-Contoured Ablation (“Custom-CAP”). Please check with the laser manufacturer to confirm certification requirements.
