  1. Special Notice for New York State: “This product is a life insurance policy and rider that accelerates the death benefit on account of chronic illness. It is not a health insurance policy or rider providing long-term care insurance subject to the minimum requirements of New York law, does not qualify for the New York State Long-Term Care Partnership program and is not a Medicare supplement policy or rider.”

  2. Chronic Illness Benefit Rider (CIBR) The Chronic Illness Accelerated Death Benefit Rider provides the policy owner with the option of accelerating the payment of a portion of the life insurance benefits available under the policy if the insured becomes chronically ill while the policy is in force.

  3. What is it? Allows policyholder receipt of a portion of their death benefit while living • What is the cost of the Rider? Upon application… No Cost* • What “triggers” eligibility for the benefit? • Unable to perform 2 of 6 ADL’s • Bathing, Toileting, Dressing, Continence, Transferring, Eating • Severe cognitive impairment *Charges are assessed at time of acceleration.
