  3. Discussion • Why? • ‘We never have any major issues’ • ‘Time consuming’ • ‘Are you serious? I only do fine needle aspiration! • ‘Who is going to check?’ • ‘I never check bloods!’

  4. Radiology Patient Safety Checklist • Relevant • Easy to follow • Documenting and ensuring our normal practice guided by appropriate new recommendations • Should not be time consuming • Completed checklist documented • Audit

  5. Which area & logistics? • All radiological procedure including e.g. aspiration, arm venography, nephrostogram? • Only those ‘more invasive’ procedure? • When and where to check?

  6. Who is responsible? • Radiologists? Consultants? SpRs? • Radiographers? • Nurses?

  7. Which checklist? • WHO surgical checklist? • RCR radiology checklist?

  8. Documentation of completed checklist • Filed in medical records? But outpatient / day case procedures may not have medical notes at the time of the procedure • Scanned in CRIS? Resource?

  9. Discussion • Radiology Consultants’ meeting • Radiology Lead Nurse and Radiographer superintendents

  11. Which area/procedure • Those requiring written consent except radioisotope therapy and breast unit interventions • Drainages • Biopsies • Major interventional procedures

  12. Who is responsible? • All staff involved in the procedure: - The operator - Scrubbed nurse - Radiographer (except US)
