Why is this important for Divisions? • Divisions are key member agencies of PCPs • Timely and opportunistic to be involved in driving local strategies to address health needs • Partnership development • Service coordination • Integrated chronic disease management • Integrated health promotion • Links with the National reform agenda

  5. Victorian Statewide Referral Form • The purpose is to provide a standardised mechanism for referral from General Practice to state funded health providers. • Reduces duplication of forms GPs have to use • Available in a number of GP software applications

  6. VSRF • There has been a 2009 VSRF review • Includes additional templates focusing on maternity, urology and orthopeadic • GPV currently working on finalising templates which will be available on the website • http://www.gpv.org.au/content.asp?cid=11,137&t=VSRF

Small grants to divisions Options and ideas • Supporting practitioners to put training into practice • PDSA cycles in practice • Supporting systematic patient identification • Effective use of available MBS items • Supporting use of local referral pathways • Online learning options eg; RACGP gplearning implementing lifestyle change module • Local/regional education and training • Other ideas???
