  3. SERVICE ENTRY Open or enter the person or organization to whom you have provided a service. Click on Service button Click on New Log Entry

  4. Service Entry Continued: This is the Service Screen Auto-Include in Stats should automatically be checked Auto-Person or Organization automatically checked Auto-Staff (your name) RIGHT COLUMN • Auto-Service ID # • Time – if you want • Duration – if you want • County – Where SE provided

Service Entry Continued: Program – Drop Down and click on MAF Type – Drop Down& click one • Asset Mapping • I&R • MDE/CAUSE Adv Referral • Mediation/Facilitation • Mentoring – all types • Outreach • Support & Education • Training • Playgroups Activity – Leave blank or look at Service Type Description Means of Contact – Drop Down (Initial Contact for that service)

  6. Service Entry Continued Status – open/closed Outcome – referred/resolved THIS IS FOR YOUR USE ONLY

  7. Service Entry: Child/Student Tab Child/Student Tab = The Child with a Disability Check Box: Consumer is in the Contact’s Household if parent Child is entered in as a person! Right Click on yellow box to find the Child’s name in person table or click “NEW” to add the Child’s name to People table. DOB, Age, Gender, Ethnicity, and Disability is entered on the Child’s Person Form and is automatically transported into the Service Form’s Demographic Information when the person is saved.

  8. Service Entry: Child/Student Person Form Do enter: • Name • Date of Birth • Age is entered automatically from DOB • Ethnicity • Gender These are the top middle two boxes on the Person Form You may want to enter the name of the parent on the Child’s Person Form

  9. Service Entry: Child/Student Tab Fund Source • Don’t Know/Not Sure • EO/Part C birth-3 • EO/Part C Transition • Not currently served • N/A • Other • Part B Preschl 3-5 yr • Part B School 6-21 yr • Part B Trans 16-26 yr • Section 504 Plan

Service Entry: Child/Student Tab • When another Service Entry is logged for a parent, click on the yellow box to find the child, double click on the child, and automatically add the demographic information. Please do this each time! • This is useful when working with a family who has two or more children with special needs.

  11. Service Entry DescriptionTab: Notes Box Notes: Double click in box and Service Date and staff members name is added automatically. This should be changed if you are entering for another person. MAKE SURE SERVICE DATE IS CORRECT Topic(s) shows when you click in the Note box. Put the major topic such as “Autism, IEP” Add more detailed notes about service provided below this

  13. Service Form: Task Tab • MAF is not using this at present time • PM can use this if want to keep track of tasks needed for the parent or referral

Documents-Attaching a Document • Must be in “Saved” format to attach • Click on “New Doc” at the bottom left to attach • Browse in your computer • (M Drive, H Drive) to find the document • Click on the document twice to “Attach Document” • Document will show in “File Name” • Add comments and Notes if wanted • Click “Save” at Upper Left to attach document

Documents-Removing, Opening • Must be in “Saved” format to attach/remove • Click on “Unlink Doc” at the bottom to remove • If you have already linked the document and you want to open it, double click on the name of the document • If you want to change the document to a different one, • click on the document once so it is blue and then click “Change Document” • the document screen will open again and you can go through the process of browsing etc.

  16. Service Form: Referrals Tab • Referrals Tab = Referrals to Organizations • You must enter MSEMP and MDE Advocacy Referrals here • Date – Make sure is service date: type in or use calendar, or change date at bottom

  17. Service Form: Referrals Tab • Left click in Agency ID box and it will turn yellow • Then right click and list of Organizations appears • Double click on Organization you referred to (MSEMP, MPAS, Local Advocate)

  18. Service Form: Referrals Tab • Select Purpose for referral • For example, you referred to MSEMP for mediation. Choose Mediation/ Facilitation in list

  19. Service Entry: Household Tab All can use this tab but information is required for United Way grant Note: Often this information is unknown. There is an “unknown” selection for each box • Household Income There is one combined income for the household.

  20. Service Entry: Household Tab Primary & Secondary Head of Household If one person head of household, fill in left column If two person head of household, use both columns • Name • Gender • Age
