HFMEA • HFMEA stand for Healthcare Failure Mode and Effects Analysis. • It is an analysis of potential failure modes within a system for classification by severity or determination of the effect of failures on the system. It is widely used in manufacturing industries in various phases of the product life cycle and is now increasingly finding use in the service industry. Failure causes are any errors or defects in process, design, or item, especially those that affect the customer, and can be potential or actual

  15. 5 Whys Analysis • The 5 Whys is a technique used in the Analyze phase of the Six Sigma DMAIC methodology. It's a great Six Sigma tool that doesn't involve data segmentation, hypothesis testing, regression or other advanced statistical tools, and in many cases can be completed without a data collection plan.

Why did it run out of gas?  - Because I didn't buy any gas on my way to home.3. Why didn't you buy any gas this morning?  - Because I didn't have any money.4. Why didn't you have any money?  - Because I spent it all last night purchasing new computer.5. Why did you spent it all your money new computer?  - Because I upgrade the options and cost me more than I expected.
