About Us With the purpose that everyone can enjoy a high-level of healthcare regardless of income, since 2012, Bive (Fundación Vive con Bienestar) facilitates access to a high quality health services for low-income Colombian families, in accordance with the ability to pay. Mission Fundación Vive con Bienestar is a social enterprise that facilitates access to high quality, efficient, and affordable healthcare services healthcare for Colombians families, with a special focus on vulnerable communities. Vision By the year 2020 Fundación Vive con Bienestar is positioned to be one one of the best alternatives for accessing health services for Colombians.   3. 2017 Management Report Similarly, the management of our partnerships for the geographic expansionof our impact from the department of Risaralda and the structuring of structuring the processes of the project unit to give continuity to the management of health projects framed in the areas of women's health and cardiovascular risk. Even so, we have a long journey ahead of us in improving the well-being of populations that we serve. Our work would not have been possible without the effort and passion for service that characterize that team that works at Fundacion Bive and the trust and commitment to our network of health and social orientation providers, our advisory board, cooperatives, and partners that have united behind our mission to bring access to high-quality health servicesto every corner of Colombia. Diana Carolina Quintero Directora Ejecutiva We are thrilled to present Bive’s work in facilitating access to high quality health services to low-income and vulnerable populations in 2017. After three years of working on constructing a model of access to health services, worthy of the rural communities based on a close service and according to their specific needs, in 2016 we impacted 15,000 people and deepened our programs in oral and visual health. During these years, we have achieved results and learned a lot, representing the consolidation of two strategic initiatives: Bive Program and the development of Social Projects in Health.   4. Our Impact Access to early diagnosis and and treatment guarantees healthier and happier lives, for low-income communities. In 2017 we facilitated access to high quality health services for 15,000 Colombians, 12,000 of which coming from rural and vulnerable communities.

Bive Program Low-cost membership that permits families to access high quality health services, at a faster rate than EPS and at a price according to a patient's ability to pay, usually at a discount of 50% of the market price. 2017 Results 50 social orientation cases. 23 successful legal advice for the restitution of health rights. 15,000 users 290 medical providers in Caldas and Risaralda 1,773 health services facilitated

Social Projects in Health Health Smiles (Sonrisas Saludables) improves the oral health of 1,250 low-income children through providing general and specialized dental treatments and oral health education through family workshops and SMS reminders to encourage strong dental habits. Coffee Smiles (Sonrisas Cafeteras) facilitates the improvement of dental health of 1,000coffee growersthrough the provision of dental prosthetic rehabilitation and oral prophylaxis. Coffee Vision (Visión Cafetera) facilitates the early detection of the common causes of preventable blindness (refraction errors, cataracts, glaucoma, and cataratas, pterygium) thus allowing for the restoration of vision, provision of glasses, and corrective surgery for 1,500 coffee farmers.
