Pre-requisites to Conducting Medicals • Please ensure that the following are adhered to while conducting a medical: • Client is carrying a letter mentioning the authorised centre(s) he is to visit. • The name of your centre features in the letter. If it does not, please do NOT perform any medical tests. • ME slip is brought by the client and mentions MER along with any other tests which are to be done • Photo ID is checked before doing the medical and identification number mentioned on the MER • Identification mark is noted in the MER

The Medical Examination Report (MER) • The MER is the only examination that is required in all categories of pre-policy medicals • It is a very critical input in the underwriting process to decide whether a policy can be issued or not. • It is a comprehensive combination of questions and examination findings to obtain the maximum amount of information about the Life Assured’s Health and Lifestyle. • However, there are instances when certain valuable information is not correctly captured • Common discrepancies noted are highlighted in the subsequent slides

If stamp missing, payment cannot be made ** ** Your ME code which is important for payment is obtained through the stamp. In case you do not have the stamp, contact your branch for the same

Summary - MER • Please check the following before dispatching the reports: • Photo ID clearly mentioned and identification no. written on MER • Identification mark noted & mentioned • All Qs in the MER form have been put to LA, singly or condensed and noted • Special emphasis on +ve history; details of such history recorded • Detailed family / personal history taken, as they constitute majority of risk factors. • All vital measurements captured. • Client’s signature, your signature and stamp have been affixed • Seal in the envelope provided & handover ONLY to our courier • In cases where reports have to be personally handed over to an official of ICICI Prudential, verify his identity and ask for a letter from an authorized person such as Unit Manager / Branch Manager. Keep the letter with you.
