  3. Training Objective & Definition • Maintain an up-to-date problem list of current and active diagnoses in EHR. • Definition: Problem List – A list of current and active diagnoses as well as past diagnoses relevant to the current care of the patient. Problem List

  4. MU Objective and Measure Maintain an up-to-date problem list of current and active diagnoses. More than 80% of all unique patients seen by the provider or admitted to the hospital’s inpatient or emergency department have at least one entry or an indication that no problems are known for the patient recorded as structured data.   6. RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: one or more face-to-face visits with the provider during the EHR reporting period Problem List

RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the Denominator WHERE: structured data is present during the EHR reporting period indicating a problem (active or inactive) on the problem list HAVING: an entered date on or before the end of the reporting period OR HAVING: a deleted date on or between the first and last days of the reporting period OR HAVING: structured data present during the reporting period that documents there are no active problems Problem List

  9. What HIM Needs to Know • Delineate the roles for managing a complete, accurate & current problem list • Educate the providers on the maintenance of a current problem list • An up-to-date problem list facilitates the correct selection of clinical indication and documentation Problem List

  10. Configuration • Assignment of correct keys • BGOZ PROBLEM LIST – allows editing of problem list, assign to all providers • APCMZ PATIENT LISTS - allows creation of patient list for specific MU measures, assign to MU coordinator and CAC’s Problem List

  11. How-to Session • Add/Edit/Delete problems via the GUI or PCC Data Entry • Running a patient list for problem list under MU report menu in PCC Problem List

  12. Policy and Procedure Review, things to considered • Who will be allow to edit problem lists • Process for addressing patient without problems, ie young adults, children • Create a workflow on how to capture this data Problem List
