  5. The Process You have decided to make the move to Newport… What Now? • WFM will send you a selection letter • A CD-150 and Estimated Relocation Worksheet will be attached • Complete and submit the CD-150 and Estimated Relocation Worksheet

  6. The Relocation Expense Worksheet • Employees – complete page 1 OMAO Admin uses an excel spreadsheet with formulas already in place to complete the remaining pages

  7. Who Does What • OAMO Admin will: • Prepare your Travel Order • Request HHG Shipment & Storage • Moving Company will: • Contact you to arrange Shipment & Storage of HHG • Deanna Stewart will: • Enter you into Prudential’s BVO Program if requested on CD-150

Check Your Travel Order Make Sure: • Your SSN has not been recorded. • Each family member who is relocating is specifically listed in block 5. • House Hunting if requested is authorized in block 15 and includes the maximum # of days. • The number of days authorized for TQSE and Temp Storage is provided in block 15.

  9. You Have Your CD-29:Begin Incurring Costs • Take Your HHT if authorized • Make the En Route from Seattle to Newport • Enter Temporary Quarters • List Your Home For Sale • Purchase a Home in Newport • File your MEA

  10. You’ve Incurred Costs:How To File for Reimbursement • Complete a CD-370 each time expenses are claimed • Obtain Approving Official Signature on CD-370 • Submit with receipts to Deanna Stewart at the WOB

A Word About Temp Qtrs • TQ is not an entitlement and will be authorized in 30 day increments less the number of days spent on a HHT • Requests for Additional Days in TQ must be made in writing to OMAO at least 10 days before the end of the authorized period • Unlike en route travel, TQ is based on exact meal costs, not to exceed the per diem rate (see the Relocation Expense Worksheet for calculating costs) • Receipts are required for any individual cost over $75.00 and for all lodging regardless of amount

  12. Selling or Buying on Your Own When you don’t use Prudential • File a CD-370 and CD-371 (both of which must be signed by the Approving Official • Include your HUD Settlement Statement
