  1. Step 1. Connect to the internet through your network or dial-up service and then go to this web address: www.eriskservices.com

  2. Step 2. From our home page, you can either click on the shopping cart or use the navigation bar to get to . . .

  3. Step 3. Indicate the submission type – NEW or RENEWAL – and your E-Mail address and then . . . . . . Click here to begin answering the application questions.

  4. Step 4. Main BAM Application page

  5. If you are a broker who is already registered to use our system *, your information will automatically appear in the application form. * If you are not yet registered with us, you can do so from our web site.

  6. Step 5. Enter the application information for the Broker, General, Financial, Prior Insurance and Prior Activities Information sections (mandatory). MAKE SURE TO ENTER YOUR E-MAIL ADDRESS.

  7. Step 6. Enter the application information for the Coverages Sections desired (EPL; D&O; Fiduciary; Crime; Technology, Media & Professional Services (Cyberliability); or Miscellaneous E&O). The product flexibility allows for the broker/client to pick and chose to apply for one or a combination of multiple coverages. Click on the coverage you seek and . . .

  9. Step 7. After completing the Application information, click on Submit Application and then click on the Submit Form button. Before sending the application, add any additional comments not included in the coverage section questions To clear all the responses you entered, click here.

  10. If this screen appears, you have completed the application correctly. An e-mail will arrive shortly containing hyperlinks to the application, indication letter and bind request form.

  11. If this screen appears, the application form you filled out is incomplete for the indicated areas. Use the BACK button on your browser to fix the problems listed above. Go back to each of these sections and review the information you entered. Most likely, something is missing from your responses.

  12. Step 8. An e-mail will automatically be sent to the e-mail address listed in the Broker Information section of the Online Application. There will be three hyperlinks included in this e-mail – one to view the application, one to view the indication letter and one to request binding coverage. Click here and view . . .
