It’s Been Done Before! • As much as 90% of all lands in Illinois and Missouri (and AL, LA, AR, IN, FL, etc.) were federally controlled for decades! With so much land under federal control, these States persistently argued they could not: • adequately fund education, • grow their economies, or • responsibly manage their abundant resources.

Summary of Included Services • Qliance provides comprehensive primary and preventive care.  That means you should call us first when you have a health care question or problem so we can help you to figure out the best way to address your needs.  Think of us as your trusted health advisor – there to listen to you, care for you, and help you navigate the healthcare system as a whole. Health and Wellness • Your Qliance provider offers preventive visits as well as lifestyle and wellness coaching.  An annual checkup is included in your membership.  Your visits may include extended consultations and personalized coaching to create positive lifestyle changes such as weight loss, smoking cessation, and stress management. Chronic Condition Management • Qliance is a great way to manage chronic conditions such as high blood pressure, diabetes, high cholesterol, asthma, arthritis, and many others.  Your provider will take the time to understand your situation, empower you to manage your own health, and keep you up to date with the most current national care guidelines.  If your condition is unstable or complicated to the point where a specialist’s care is needed, your Qliance provider will refer you to a qualified physician who will assist in your care. Specialist Care Coordination • Should you need to see a specialist, your Qliance provider will coordinate your referral, communicate with your specialist, and work with you to make decisions about and follow through with your care plan.  If you wish to see a specialist, please make sure to communicate with your Qliance provider first so that we can help you find the best care and make sure your care is appropriately coordinated.

Urgent Care • We provide same or next-day care for urgent medical issues such as sprains, respiratory illnesses, cuts requiring stitches, urinary tract infections, fractures, and more.  For your convenience, we have on-site or near-site x-ray available at no additional cost to you.  If you are not sure whether your condition is something we can take care of at Qliance, give us a call and we will help direct you to the best place to get it addressed. Hospital and Emergency Room Visit Coordination • In the case of serious illness requiring emergency or hospital care, your Qliance provider will communicate with the hospital, send over any relevant records, and coordinate your care during and after discharge.  If you have a serious emergency requiring immediate medical attention, call 911 and then either call Qliance or have the hospital personnel call us to let us know so we can participate in your care.  Most of our providers coordinate hospital and emergency care remotely, working with other doctors and care providers over the phone and touching base with you and/or your family members.  Some of our providers maintain hospital privileges and provide direct, bedside care and coordination (at Swedish Hospital First Hill Campus only). Laboratory Tests • Qliance offers some basic onsite laboratory testing at no additional charge (such as pregnancy testing, strep throat testing, HIV screening, and others).  For all other tests, Qliance sends samples to our partner reference laboratories for processing.  If you would like to use your insurance to pay for your tests, we will forward your information to the laboratory to be billed directly to your plan.  If you would like to take advantage of discounted cash pricing, you can pay directly at the time of service and enjoy discounts of up to 75% off of regular pricing.  All testing and any associated costs will be discussed with you prior to ordering and processing those tests. Office Procedures and Supplies • Qliance providers perform a number of in-office procedures, including skin biopsies, stitches, wound care, casting/splinting, EKG’s, and more, all included in your membership.  If needed, we can also provide you with a selection of supplies (such as braces, splints, and crutches) at no additional cost.  Samples obtained from any procedures (such as skin biopsies) may need to be sent to the laboratory for further analysis, which will be charged according to our laboratory test procedures (see above). Medications and Prescriptions • Prescriptions can be sent to the pharmacy of your choice, including mail order. Vaccines • We carry the most common childhood and adult vaccines, including the flu vaccine when in season.  In Washington State, all children’s vaccines are provided free of charge.  Adult vaccines are charged at cost (or to your employer or trust if you are in a work or union-sponsored plan).  We also provide some travel vaccines – talk with your Qliance provider if you are planning a trip out of the country so that they can advise you on proper preparation for travel. • Qliance does not provide maternity/prenatal care, specialist care, advanced imaging, emergency services, or complementary and alternative care such as acupuncture, chiropractic, massage, or naturopathy.  However, we will be pleased to help you find the best source for these types of care as needed.
