2. WHO CAN RECEIVE ACCOMODATIONS? The following disabilities are just some of those that could qualify a student for accommodations via The College Board • Blindness/Vision Impairment • AD/HD (formerly known as A.D.D. /A.D.H.D.) • Learning Disabilities • Deafness/Hearing Impairment • Certain Medical Conditions • Certain Physical Disabilities

3. ELIGIBILITY • In order to use accommodations on College Board tests, accommodations must be approved by the College Board's Services for Students with Disabilities • Use of accommodations without prior approval will result in the cancellation of test scores • Once approved, students remain approved and do not have to apply again when they apply for another College Board test

4. WHY SUBMIT THROUGH AN SSD COORDINATOR? • Submitting without the assistance of an SSD Coordinator is a complicated process. If a student or their guardian(s) submits a request without the assistance of the SSD Coordinator at Durfee High School, documentation of the disability is required and is subject to a thorough review via a national panel of experts • Contact the Durfee High School Guidance Department to get assistance from the SSD Coordinator. 508-675-8100 x1200

5. HOW DO YOU APPLY FOR ACCOMODATIONS WITHOUT HELP FROM THE SCHOOL? • If a student or parent prefers to apply for accommodations without assistance from the school, they should contact College Board for an eligibility form

6. REQUIRED DOCUMENTATION • State the specific disability • Diagnosis should be made by a person with appropriate professional credentials (MD, Ph.D) • Be current • In most cases, the evaluation should have taken place within five years of request • For psychiatric disabilities, evaluation must be within 12 months of the request for accommodations • For visual disabilities, documentation should be within two years, and for physical/medical, an update must be within one year from the time of the request • Provide relevant educational, developmental, and medical history.

7. REQUIRED DOCUMENTATION (cont.) • Describe comprehensive testing and techniques used for diagnosis. Include test results with subtest scores (standard or scaled scores) for all tests. See Documenting Specific Disabilities for a list of frequently used tests and what they measure • Describe the functional limitations.Explain how the disability impacts the student’s daily functioning and ability to participate in the test • Describe the specific accommodationsbeing requested on College Board tests, including the amount of extended time required or the maximum amount of time the student can be tested in a day. State why the disability qualifies the student for such accommodations on standardized tests • Establish the professional credentials of the evaluator (for example, licensure; certification; area of specialization)

8. DOCUMENTING SPECIFIC DISABILITIES • VISIT COLLEGE BOARD FOR ADDITIONAL SUPPORT ON DOCUMENTING SPECIFIC DISABILITIES SUCH AS: Learning Disabilities, ADHD, Hearing Impairments, Psychiatric Disorders, Head Injury/Traumatic Brain Injury, Visual Impairments, Physical/Medical Disabilities, Autism Spectrum Disorders, Speech and Language

9. FINAL DETERMINATION OF ELIGIBILITY • When College Board determines student eligibility, he or she and the school will receive an Eligibility Letter • The student is assigned an SSD Eligibility Codethat is used when communicating with the College Board and when registering for test(s)
