  3. Omartose • Na heup en knie meest voorkomende artrose • Pijn en stijfheid in alle richtingen • Veel invloed op ADL, zelfstandigheid

  4. Diagnostiek • Leeftijd • Pijnlijke rotaties, stijfheid en crepitaties • DD met frozen shoulder • Rontgenonderzoek • Echo (status rotator cuff)

  5. Therapie • Conservatief - NSAID’s, - fysiotherapie, - intra articulaire cortisoninjecties (steeds vaker echo geleid)

  6. Therapie • Operatief - artroscopie (‘wash out, micro fracture’) - interpositie graft (controversieel) - schouderprothese

  7. Resurfacing prothese • Cementloze schouderprothese zonder steel • Zoveel mogelijk behoud van bot, opties open voor toekomst • Anatomisch • Lange follow-up (> 20 jaar) met 80% succes Mr Copeland

  9. Positionering Patient • Strandstoel ligging • Plexus blokkade • Arm op armsteun of ‘robotarm’

  10. Superolaterale Benadering(volgens MacKenzie) • Alternatief is deltopectorale benadering • MacKenzie beter zicht op glenoid • AC beter bereikbaar • Distaal uitbreiden beperkt (welke structuur?)

  11. Superolaterale Benadering(volgens MacKenzie) • Laterale clavicula resectie • Acromion osteotomie (consequenties voor nabehandeling • Teugelen subscapularispees • Bicepspees tenotomie • Subacromiale decompressie

  12. Humerus • Verwijderen osteofytenrand • Prothese in natuurlijke retroversie (20-35º) door collum anatonicum te volgen • Botslijpsel opgevangen en gebruikt als autograft

  17. ‘Biological Resurfacing Glenoid’ • Glenoidcomponent zwakste schakel (loslating) • Fibrokraakbeen op gelnoid • Microfracturing met chondropick

  18. Check Retroversie

  19. Autograft

  20. Aandrevelen

  21. Sluiten cuff en acromion • Tijd voor nemen • Subscapularis pees op tuberculum minus terughechten met arm in 30º exo • Acromion met transossale hechtingen

  22. Overdracht aan fysiotherapeut(e) • Immobiliser sling • Body belt voor 1 dag, daarna af om stijve schouder te voorkomen • Invullen instructievel op OK met alle bewegingsuitslagen, type benadering en bijzonderheden/com-plicaties

  23. Rotator cuff artropathie • Proximale migratie humeruskop bij massale cuffruptuur • Glenohumerale artrose • Hemi of resurfacingprothese verbetert niets aan functie
