  1. UW Radiation Oncology CSI outcome Patty Sponseller,CMD August 2012

  2. August 2011 Introductory CSI meeting held August 2011 Very busy time in our department Emergent patients most days

  3. Dosimetry • Increased workload with burdensome software • Also responsible for scheduling patients FotoSearch

  4. Outdated software

  5. PRISM Planning

  6. PRISM use • Second check for Monitor Units each field • Neutron beam planning

  7. Monitor Units • What is a Monitor Unit? • Why check?

  8. Central axis-CAX

  9. Shape

  10. Central axis-CAX Depth in patient

  11. www.uthgsbsmedphys.org

  12. PRISM Calc tool • Required manual data entry for most parameters • Make a mistake? Start over • Inefficient and time consuming

  13. Commercial MU check program

  14. Workload efficiency! • Efficient • Friendly • Windows based-easy to support

  15. Dose verification • In vivo dosimetry directly monitors the radiation dose delivered to a patient during radiation therapyavoiding a misadministration. • It allows comparison of prescribed and delivered doses and thus provides a level of radiotherapy quality assurance that supplements port films or cone beam CT and Monitor Unit double checks. • AAPM Task Group 62 Radiation Products Design

  16. Dose is calculated from isocenter to skin surface

  17. At skin surface diode is placed prior to RT

  18. Change workflow for diode readings Monday from manual entry in Mosiaq assessments to RadCalc

  19. 62 DIODES incorrectly reported MU: The DIODE box malfunctioned. ACTION Eric will ask George to request emergency capitol to replace/fix the diode box. [TRACK] request submitted to physics lead Eric will bring up at Monday meeting Followup on 7 JUN to see if ordered ? new diodes placed in C box not replaced. COMPLETE Review at next July 15 physics meeting New DIODE box on order 102 Received 9 Fx prior to Diode ACTION Lulu will discuss with therapists why they didn?t diode for 9 fx [TRACK] create report is repeats 156 Diode not reviewed and therapists did not diode one field ? Matt will look into this   [TRACK] ? RADCALC for diodes. 179 (221) Transcription error in Dosimetry on diode placement - Caught on chart check [TRACK] ? Agenda item for physics meeting to use RADCALC for diode entry. 196 (240) QCL sent but DIODES not signed found at new diode check. ACTION move DIODE entry to RADCALC (Eric) ? review next week George S. agreed to move forward with this. Deciding on training plan for 6 August. 221 (269) Diodes not approved / final physics check ? RADCALC for diodes?? [TRACK] Discuss at physics meeting [Eric] issue 196 implementation plan 246 (306) Dosimetrist did not label the field name/# right in diode assessment page (correct expected reading, but old field name/number prior to block change). Will be (partially) resolved with the use of RadCalc for diode checks. [TRACK] 247 (294) Diode energy entered wrong – fixed with RADCALC 248 (301) Diode reading high but not redone – [TRACK] found physics chart check

  20. Diode placement Place diode here

  21. Early 2012 neutron planning in Pinnacle

  22. Change in patient scheduling • Once plan was completed Medical Dosimetrist would schedule patient • Now patient leaves CT simulation with verification appointment • Chief Therapist

  23. Changes have made a huge impact
