Background/Partnership History • MCC was founded in 1978 out of a town hall meeting in the GLBT community • Legacy was founded in 1981 as an STD clinic for the gay community and is now an FQHC serving the entire community • There has been a long history of collaboration on projects for HIV prevention, breast cancer outreach and screening, senior services

  3. About Our Program • Integration model – MCC and LCHS are providing integration at both sites. MCC providing substance abuse and case management at LCHS, LCHS providing screening, wellness and nutrition at MCC. • Strategies used to incorporate primary care - health screenings, wellness classes and events, nutritional counseling • Enrollment target – 60/year

About Our Program • Special populations served – LGBT and HIV • Urban setting • Wellness services offered – yoga, fitness room, peer resiliency series, tobacco cessation, healthy eating series, and psychoeducation of SMIs • Use of peers – resiliency series and consumer advisory panel • EHR vendor – MCC: “home grown”, LCHS: Centricity

Successful Strategies • Designing a customized, home grown electronic health record • Step by step involvement of clinical staff to design a product customized to our needs • Results – making the system relevant to the clinical staff, capturing the data that can help our clients • Enrollment/Reassessment • Understanding each system and how they can work together • Developing communication tools • Sustainability • Understanding each others billing and financing structures • Planning for 2014

Plans for the Future • Sustainability • Clinical – working our incorporating the health assessment into MCC behavioral health assessment and electronic records, testing record exchanges • Financial – credentialing the substance abuse treatment program under FQHC • Accountable Care Organization activity • Exchanging records through the Health Information Exchange • Transmitting public health surveillance data and lab orders
