Summary of Report Format Changes • Changes were prompted by customer suggestions and comments • Include: • Addition of text box for Consultation Summary. • Re-location of Clinical Significance/ Transfusion Recommendationbox to the beginning of the report. • Bold border around Consultation Summary and Clinical Significance areas.

  3. Summary of Report Format Changes • Continued • Addition of Date/Time lines for supervisory and medical director reviews. • Addition of Enzyme as an enhancement media option. • Addition of Patient Identifier information to multiple pages of the report.

  4. Consultation Summary • This area will contain an overview of the test results. • Will include ABO/Rh. • Will list antibodies identified in the plasma and eluate. • Detailed explanations will be contained in the body of the report

Clinical Significance/Transfusion Recommendations • Information has been moved to be more accessible to technologists. • Includes Transfusion Recommendations, if applicable. • Includes Clinical Significance of antibodies identified, if applicable. • Includes Clinical Significance of titer results, if applicable.

Bold Outline Box • This easily located box will contain all the pertinent information needed to make transfusion decisions. • Includes: • Consultation Summary • Clinical Significance/Transfusion Recommendations • Details on the antibody identification procedure will be contained in the body of the report

  13. Patient Identification • This information will display at the top of subsequent pages of the consultation report. • Includes: • Patient name • Date of Birth • ID number • Sample Date

  15. Enzyme as Enhancement Media • We can now choose Enzyme as an enhancement media • Similar to PeG, LISS, Saline, Gel • Will eliminate some of the extra “wordiness” from the text • May still comment if a particular antibody reactivity is destroyed or enhance

  17. Date/Time Entries for Reviewers • This information was provided as a result of a client hospital suggestion. • Will allow better understanding of the approval routing of our consultation reports.
