  1. Information Technology My Plan and Orders NextGen 5.7 KBM 8.1

  2. If you need to order additional point of care tests or immunizations, you can return to the SOAP template. • There are links to the left side of the assessment grid. • This is where you can order labs, office procedures or patient instructions based on diagnosis.

  3. My Plan – Order Sets • Single left click the diagnosis you would like to create a plan or order for, then click in the check box for the items you would like to order. This loads a pre-determined set of orders for lab & diagnostic testing, office procedures and meds and patient instructions. • You can task other staff members to perform the tests by clicking on the Task button. • When you have completed the orders for the selected diagnosis you must click on Place Order. • You can then order on an additional diagnosis.

  4. You can also add tests, diagnostics, medications that are not listed on the template by click on the blue drop down arrow next to the text “Click arrow to choose”. • A pick list will display with additional tests for that category.

  5. Use/Modify System Order Sets • Click on the diagnosis to view the associated order set. • Select the appropriate orders by placing a checkmark in the box to the left of the order. • To add an order item, click the button at the bottom of each section and search and select the appropriate item. • Continue the process through each section as appropriate.

  6. Add/Modify Instructions • Instructions section – used to document general instructions or education given to the patient. Place a checkmark next to the instruction you would like to use for the patient. To add additional instructions: • Click the dropdown arrow next to Select type. • Choose Instructions. • Click the blue down arrow next to • Enter the appropriate text. Place a checkmark next to the instruction to include it in your orders. • Click the button to add the instruction to your picklist of instructions. Although you must choose a specific number, the instruction may be used in any number field.

  7. Add/Modify Instructions • Click in the instructions text box to bring up the picklist. Click here to bring up picklist

  8. Followup Visit/Referral • Follow-up visit/referral section – check the appropriate box and click the blue drop down arrow to open the Order Timeframe pop-up. • Enter the appropriate follow up timeframe, reason, etc.

  9. Order Sets • When all orders are selected, click the button. Note the template is cleared by the Place Order process. To create custom order sets: • Highlight the appropriate row in the Today’s Assessment or Problem Focused Assessments grids. • Either a pre-customized set or a starter set will populate. • To delete an item, click the button and select a line number from the picklist. You may replace the deleted item with another. • Continue the same process for each section. • When completed, click the button in the upper right hand corner of the template.

  10. Custom Order Sets To use saved order sets: • Click the button next to My savedin the upper right hand corner. • Choose the appropriate saved order set from the picklist. • Click To use others saved: • Click the radio button to open the picklist of order sets saved by other providers. Note that these will be specific to the selected diagnosis. • Click the orders you wish to use. • Click the button. Diabetes NOTE: You may make modifications to order sets from other providers and click to add as your saved set.
