Minneapolis Health Care for Homeless Veterans Outreach Outcomes for FY 2007 311 full intakes with an average of 7 visits 41 partial intakes with an average of 3 visits 74% in shelter at time of intake 13% referred by VAMC 88% of clients have psychiatric or substance abuse disorder/s 97% of clients referred/provided services by another MH provider post HCHV contact

Minneapolis Health Care for Homeless Veterans Transitional Housing through GPD Programs For FY 2007 we had 2 programs with 23 total beds 45 clients enrolled for FY 2007 34 placed during FY 2007 31 discharged during FY 2007 Average of 30 appointments per client with HCHV staff 97% participated in work therapy < 2% of transitional clients received no additional services from VHA Mean stay 195 and Median 185

    8. Minneapolis Health Care for Homeless Veterans Characteristics of FY 2007 transitional discharges 72% some services before AND some services after 25% successful discharge (regular, planned discharge to next level of care)

Minneapolis Health Care for Homeless Veterans HUD-VASH permanent SUPPORTIVE housing program 35 vouchers FY 2008 35 vouchers FY 2009 Partnership with HUD, Minneapolis PHA, veterans, and VHA 3 veterans have completed intake and screening process, have vouchers, and leases pending 2 HUD-VASH CM pending hire goal to have staff on board before end of this calendar year

Minneapolis Health Care for Homeless Veterans Advocacy and Community Partnering to provide services to homeless and near homeless veterans: STAND DOWNS represented at 4 different sites this year CHALENG yearly report that is coordinated by HCHV and represents community, veterans, and VA views PROJECT CONNECT participant provider PARTNERING with individuals, community, county, state, and federal programs to provide support and information to assist veterans
