  2. Stage I • Disease is located in a single lymph node region or a single extranodal organ. Extranodal organ refers to any organ outside of the lymphatic system, for example the lungs. Lymph node Diaphragm

  3. Stage II • Disease is present in 2 distinct locations, both affected areas are on the same side (above or below) of the diaphragm. The affected locations may be 2 distinct nodal regions or one nodal region and one extranodal organ. Lymph node Diaphragm

Additional Information • Following allocation of a stage, certain letters are added to signify additional useful information about the disease: • The letter B is appended to the stage if the patient has a fever >100.4°F, night sweats, or loss of at least 10% of body weight in the 6 months prior to diagnosis (eg, stage IIB). These symptoms are commonly referred to as B symptoms. If these symptoms are absent, the letter A is appended, (eg, stage IIA). • If tissues close to, but distinct from the major lymphatic regions are affected, the letter E, (for extranodal) is appended to the stage, (eg, stage IIIE). • If the spleen is involved, the letter S is appended to the stage, (eg, stage IIE or IIES). • If bulky disease is present, the letter X is appended to the stage, (eg, stage IIIX). Bulky disease in MCL is typically defined as disease >10 cm in diameter, or other indications of high disease burden such as high LDH, and lymphocytosis.
