  11. Teaching congregations how to live healthier lifestyles…

  12. And giving them a new song to sing!

  13. So, who will pay for the “voice lessons”? • Hopefully, the NC Health and Wellness Trust Fund Commission (HWTFC) will “pay”! • An application for the Elimination of Health Disparities Initiative was submitted March 1 by the South Central Public Health Partnership. • Requested amount - $685,348.00 (for 3-yr. period) • Award notifications will be announced in May.

  14. What makes the Sparrow Project unique? It is the first… multi-county, cardiovascular disease, stroke education, & prevention program targeting at-risk African American churches.

The Robeson County Health Department will Serve as the lead agency and fiscal agent for the Sparrow Project, providing project supervision, oversight, and linkage to community resources.

And Who is included in the “sparrow’s nest”? • Target population consists of African American adults ages 18-64 living in the 13-county region. • Approximately 1,200 members of the target population will be enrolled in program over the 3-yr. period. • Estimated that 5,000 more will benefit from social marketing and the diffusion of CVD reduction education in the community.

  17. the “nest” continued… Proposed project collaborators will include 3-4 African American churches in each county, providing access to hundreds of members of the target population.

And why are they included? • Results of 2003 regional health assessment indicated the following: • 38.5% of African American respondents reported physical inactivity • Reported limited consumption of fruits and vegetables • Tobacco use rates exceed state averages, with current smoking rates in Cumberland County approaching 30% among African Americans.

  19. how will we get the church members’ health “in tune”? Project components will include the following: • CVD/Stroke Education Workshops • Screening and Risk Assessment • Exercise Regimens

  20. Health “in tune” Continued… • Nutritional Partnerships • Regular community • events focused on healthy • lifestyles and community- • based exercise campaigns • Maintenance (Booster) • Sessions • Policy-Level Interventions

Who are the project conductors? • A staff of three Health Educators and a Program Director will “conduct” the Sparrow Project, and the intervention activities. • The three Health Educators will: • Be assigned a 3-4 county region • Identify and recruit local churches in their region • Train lay health advisers in each church • Work to implement all program objectives.

  22. Conductors continued… • Program Director • Will be a master’s level health care professional • Supervise program staff • Assist the HWTF with the social marketing campaign • Prepare all reports • Ensure all the program objectives are met.

  23. Evaluation • The Sparrow Project will employ both internal and external evaluation. • The Program Director will work closely with staff from NCCU on implementing evaluation measures and data collection. • Educational and preventive objectives will be evaluated.

Key Indicators (Education) • Key indicators of the educational objectives will include: • Percentage of participants who know all the symptoms of a heart attack • Percentage who know all the symptoms of a stroke • Percentage who demonstrate functional nutritional literacy • Percentage who demonstrate knowledge about appropriate cholesterol levels • Percentage who recognize “good” blood pressure levels

Key Indicators (prevention) • Key indicators of the prevention objectives will include: • Baseline data on exercise rates, nutritional/ diet patterns, cholesterol monitoring, and smoking will be collected pre-project implementation. • Data on these indicators will then be collected annually to gauge programmatic success.

  26. sustaining the sparrow project… • Seek support from private foundations • Work with city councils and county commissioners to recognize health disparities and adopt policy recommendations • Consider the development of a fee-for-service model to develop revenue for un-insured members of the target population.
