Information Fluency The 5 A’s of Information Fluency • Ask: Question why the patient is demonstration the symptoms; Use assessment skill for the collection of subjective and objective data • Access: Look up results of tests that have been completed; Review patient’s medication list & allergies • Analyze: Use critical thinking to determine nursing problem list and compile interventions • Apply: Carry out the interventions • Assess: Review effectiveness or plan of care and adjust accordingly
