  7. Measurement of Safety • Safe man hours without Lost Time Accident (LTA) = number of employees X no. of working hours , they worked safely. • If any LTA occurs, the safe man hours is reset to zero.

Commonly used words in safety • Hazard – Potential to cause harm to people & equipment. • Risk - The likelihood of such occurring. • Risk assessment – Risk in executing job is assessed before any job is started. • Control measures – The precautions taken for doing a job safely, based on risk assessment.

  10. Hazard in Construction Industry • Fall of material from height. • Fall of men from height. • Cave- in. • Failure of cranes & escalators. • Manual handling- Back Injury. • Skin allergy – Occupational Disease.

  11. Hazard in Process Industry • Physical hazards such as trip, slip and fall. • Fire hazard. • Chemical hazards • Electrical hazards • Mechanical hazard

  12. Re-classification of safety hazard. • Equipment and job related. • Tool related. • Area related.

  13. Risk study • Consider an activity to be assessed. • Breakdown in to small steps. • Identify the hazard • Analyze the effect of hazard • Study , how many people are affected. • Apply appropriate control measures.

  14. Probability • Frequent. • Probable. • Occasional. • Remote. • Improbable.

  15. Severity • Catastrophic:Death or system loss or damage. • Critical:Severe Injury and occupational Illness and severe system damage. • Marginal:Minor injury and occupational Illness and minor system damage. • Negligible:Less than a minor injury or and minor system damage.

  16. Risk Rating Risk Rating = Probability X Severity

  17. Methods of Risk assessment • Hazard & Operability studies ( HAZOP) • Hazard Identification ( HAZID) • Hazard Analysis ( HAZAN) • Preliminary hazard analysis (PHA). • Fault Tree Analysis (FTA). • Failure mode and effect Analysis (FMEA)

  18. Stages of Hazard Analysis • Design phase • Construction / procurement / installation stage. • Integration / test / check out phase.
