  2. Influenza symptoms • Fever plus one or more of… • Cough • Sore throat • Muscle aches / joint pain • Weakness / fatigue • Headache • Diarrhea • Vomiting

  3. Why is this important? • By self-reporting flu-like illness during a pandemic situation, you will help protect the health of our community by helping Western monitor absentee rates.

  4. How self-reporting works • Temporary measure, starting October 5 • Report flu-like illness online using the Student Center (student.uwo.ca) ASAP • Absence Notification can be found under Personal Information on Student Center

  5. How self-reporting works • Report illness start and end date • Allowed to back-date up to 10 days • Explanation required when late reporting

  6. How self-reporting works • Place check-mark in appropriate box to request academic accommodation • Type of event/assignment missed • Faculty decides what type of accommodation will be provided
