Emergency Equipment • Be aware of: • Emergency colour codes -refer to RACE card • Fire extinguishers, fire blankets • First aid kits • Spill kits (chemical, radiation, biohazard) • Mains gas/air shut-off • Hazardous chemicals stored • Emergency contacts • Escape routes • Assembly sites 04/2012;

  4. Emergency Evacuation 1.Code Orange – Block E 2. EWIS system – Monash Buildings – East Wing - Alert tone(Beeping) - Prepare to evacuate - Evacuate tone(Whooping) - Evacuate the building 3. Leave via nearest safe exit / Follow instructions of Zone/Floor Wardens - Walk - do not run - Do not use lifts 4.   8. OH&S Act 2004: Key Principles All people – staff, students, visitors, contractors - should have the highest possible level of protectionagainst risks to health and safety. Those who manage and control workplaces are responsible for eliminating risks,or for reducing risks as far as reasonably practicable. Employers must be proactive in promoting health and safety in the workplace. Information and ideas about risksand how to control them must be sharedbetween employees and employers. Employees’ views must be considered. Employees have an obligationtocooperatewith employers. Employees are entitled to be representedin relation to the management of health and safety issues. 04/2012;

PHI OH&S Committee conforms to the OH&S Act 2004 membership drawn from major areas of the workplace assembles a group of knowledgeable professionals to consider hazards and risks, and management of OH&S issues Roles of the Committee: raises awareness of OH&S issues provides a consultative forum for development, discussion and implementation of OH&S policies and procedures facilitates discussion of preventative or corrective actions in response to reports of hazards and incidents 04/2012;

Health and Safety Representative (HSR) and Deputy HSR HSR is the employee representative•elected for a term of 2 years by the members of the established PHI designated work group (DWG)•has a range of statutory rights under the OH&S Act 2004• must be consulted by the employer prior to making changes in the workplace that will/may affect the OH&S of employees, and deciding about risk controls and procedures for resolving OH&S issuesDeputy HSR• acts only in the absence of HSR 04/2012;

Role of PHI OH&S Officer • promote OH&S awareness• provide information and advice on local health and safety issues • assist in formulating and implementing OH&S policies and procedures• assist with risk management• assist in investigation and reporting of incidents, injuries and hazards• liaise with senior staff about OH&S management• consult with HSR on OH&S issues• perform workplace inspections and ergonomic assessments• provide OH&S induction and training 04/2012;

OH&S Responsibilities of Staff/Students • Ensure the safety of themselves and others by acting with care. • Comply with OH&S policies and procedures, and adhere to all safe work practices issued by PHI. • Maintain awareness of, and heed, all safety signs and notices. • Report incidents, hazards or near-misses to supervisors/OH&S Officer/ LATS Manager immediately, and submit a Hazard and Incident Report. • Know the locations and use of emergency equipment: - first aid kits - chemical spill kits, biohazard spill kits, radiation spill kits - eye wash stations, emergency showers - fire extinguishers. • Know the names and locations of: - OH&S Officer, Biosafety Officer, Radiation Safety Officer - HSR, Deputy HSR - First Aid Officers, Zone Wardens.IF IN DOUBT - ASK! 04/2012;

  16. • Report all hazards, near-misses, accidents and injuries within 24 hours. • Use Hazard and Incident Report form –on PHI intranet, near First Aid kits Near-miss – an incident where no injury or damage is sustained, but which could have resulted in personal injury, incapacity, death, property damage, or environmental damage  Accident – damage to property or environment Injury – harm to people Incident – all of the above. • Report any faulty equipment and facilities – follow equipment isolation and tagging procedures (on PHI intranet). Waste Disposal • All employees are responsible for correct disposal of workplace-generated wastes under the Environment Protection Act (Vic) 1970. • Waste streams at PHI are: - general - recyclable - clinical (biological) - chemical - radioactive. • Handling and disposal of many wastes carry significant hazards and are thus OH&S issues. • Refer to Waste Resource document and postersfor all correct wastedisposal procedures. • PHI has a Waste Management Officer to oversee practices. 04/2012;

PHI Basic Safety Rules • Within PHI: • Do not smoke. • Do not run. • Do not obstruct nor prop open fire/smoke doors. • Report all hazards and incidents. • Supervise children under 16 years brought into PHI at all times, andallow them in reception and tea room areas only. • Obey all directives from PHI management. 04/2012;

PHI Basic Safety Rules • Within PHI laboratories, including common laboratory areas/facilities: • Do not handle, store or consume food or drink. • Do not apply cosmetics. • Wear appropriate clothing and footwear with closed toes and heels. • Wear closed laboratory coats/gowns. • Wear suitably protective gloves when handling infectious/ hazardous material. • Wear safety glasses or face shields where necessary. • Clean up spills immediately. • Dispose of waste correctly. 04/2012;

PHI Basic Safety Rules • Within PHI laboratories, including common laboratory areas/facilities: • Keep work and communal areas clean and tidy. • Do not use equipment without permission or prior training. • Remove gloves before operating door handles, telephones and computers. • Remove gloves, laboratory coats/gowns and wash hands when leavingthe laboratory. • Comply with conditions of use for radioactive isotopes and GeneTechnology Regulator (OGTR) guidelines where appropriate. • Turn off equipment when not in use. • Close doors and turn off lights when leaving. 04/2012;
