Serious case reviews 8 died by the hands of others: 5 were murdered by family members, 2 died through the ingestion of methadone, 2 died in house fires 1 died from starvation, 1 through alcohol abuse 5 committed suicide (not according to coroners) 12 were sexually abused 4 sustained serious injury 1 murdered her abusive father

  4. Serious case reviews None were on the register at the time of the incident. Only 3 had been. 1 was a looked after child, 1 was receiving respite.
