  7. Patient Education • Appropriate level -All teaching must be tailored to the learner -Patients ability to read, see hear and understand the language -Alert the team to barriers to learning

  8. Patient Education • Reinforcement -most people need to hear a concept 5-8 times before they learn it • Belief -individuals must believe that what they are being taught is true and will benefit them

  9. Self Management • Avoid non-compliant or non-adherent • Follow treatment plan • Take medications • Follow treatment regime • Follow diet
