  5. Inspection of the internals of a HP Separation Vessel. The inspection tool used in the initial job is not Intrinsically Safe. Create and assign Risk assessment and print work permit (see scenario 2)

  19. Control of Work requirements • This work permit fulfils all requirements: • Level 2 Risk Assessment • Gas Test by a competent person • Regular monitoring by a competent person • CSE Entry control person

  26. Re-Instatement 1 Can only re-instate if dependent jobs are also finished

  27. Re-Instatement 2 Removal is also controlled by printouts and 4-eyes principle

  28. Nitrogen leak test Set this up as a separate order operation “Nitrogen leak test” Assign new RA , perform gas test as shown above.

  29. Lessons learned 1 We have lessons learned functionality with every "complete ok" and "complete not ok" status setting.
