Internal Allocation Meetings (IAM’s) • Held fortnightly • Reviews all referrals (professional / self / SAM’s) • Information Search conducted prior to meeting • Function: • Allocates to a worker • Re-directs out to others or SAM’s • Reviews case progress • Attended by: • Centre Manager • PIW’s • FSW Supervisor • Health Team Leader • Invited partners e.g. CAB, SALT

Service Allocation Meeting Support for professionals FSW Care Pathway Allocation at IAM’s PIW to support Background Information Home Visits Short term package of advice, guidance or support Common Assessment Family Support Meeting to develop a co-ordinated care plan which is reviewed Family Support Plan Lead Professional (if appropriate)

  8. Support Plan • Looks at each area of 5 outcomes • Assigns a level under each outcome • Reviewed at each Family Support Meeting • Plan stays open until all outcomes at Level 1 • Measurements can be taken of impact across all outcomes
