  3. Start-up The MNT Assistant is installed from a CD. The installation puts a convenient start-up icon on your desktop. Just click on it and the control panel will appear. Opening the MNT Assistant can be made more secure by adding a password or assigning user access. www.MNTnorthwest.com

  4. The Control Panel The control panel is your gateway to MNT data management and record keeping. www.MNTnorthwest.com

  5. The Control Panel The buttons in the left column allow you to edit and view MNT patient information. www.MNTnorthwest.com

  6. The Control Panel Buttons in the right column allow you to access administrative data for managing behavioral objectives, physician, therapist and insurance company information. www.MNTnorthwest.com

  8. The Patient Profile By clicking on the “Patient Profiles” button, you may add or edit patient information. www.MNTnorthwest.com

  9. The Patient Profile Here you enter from the MNT Intake form, your patient’s healthcare provider, payer and personal contact information. www.MNTnorthwest.com

  13. The Patient Profile Click on the “Insurance” tab to view or edit payer information. The information on this tab helps to track reimbursement sources so you can easily complete claim forms. www.MNTnorthwest.com

  15. The Patient Profile When you are all done entering the MNT Intake form, you can print this information for your patient’s chart. Just click on the report button to preview the basic patient profile information. www.MNTnorthwest.com

  16. Session Notes When you are ready to enter information about this person’sMNT session, click on this button to record your data and session notes. www.MNTnorthwest.com

  17. Session Notes In this form, record your intervention strategy, time with the patient, CPT code, physician’s, diagnosis, SOAP, goals, materials distributed, nutrition Rx, next steps, profile survey and clinical data. www.MNTnorthwest.com

You can also prepare tailored patient handouts and charting documents. Click here to enter the responses from the MNT Profile questionnaires, including personal and family health history, lifestyle habits and functional assessment results. www.MNTnorthwest.com

  25. MNT Profile Questionnaires Here you enter the MNT Profile questionnaire responses and print the MNT Patient Profile for your chart. Use the tabs to select the appropriate questionnaire. Data entry is quick and easy. www.MNTnorthwest.com

  26. MNT Clinical Test Results Clinical test results, such as height, weight, blood pressures, and laboratory results collected during or for this session may be recorded in the Clinical Test Results window. www.MNTnorthwest.com

  27. MNT Clinical Test Results After you enter the patient’s testing data and goals or target values, you can print a report suitable for patient education. After typing in the patient’s height and weight, the BMI is calculated for you. www.MNTnorthwest.com

  28. MNT Clinical & Nutrient Goals Before you are ready to print an charting or educational report enter the patient’s goals or target values for your patients testing results. To supply targets or goals for the testing data, go to the ‘View/Edit Goals’ window. www.MNTnorthwest.com

  30. MNT Clinical Test Results After you enter the patient’s testing data and goals or target values, you can print a report suitable for patient education. Choose the appropriate report button to preview and print a clinical test report. www.MNTnorthwest.com

  32. MNT Dietary Analysis Results You may also enter the results of a nutrition analysis. This may be from a 24 hour recall or diet record. Just click on the Dietary Analysis Results button. Click here to go to the Dietary Analysis Results form. www.MNTnorthwest.com

  34. Nutrition Recommendations Following your MNT session you will be able to create a tailored handout to reinforce your recommendations. Select the ‘MNT Recommendations’ button to prepare your educational report. www.MNTnorthwest.com
