H1K CCP Outcomes • Move to permanent housing within 60 days of enrollment • Remain Housed for at least 12 months • Improve Self-Sufficiency score • Access to sufficient resources to meet basic needs (sub-set of self-sufficiency score) • Connected to medical home within 60 days of enrollment • Income at SSI level ($850) or above within 180 days of enrollment • Connected to behavior health services, as needed, within 90 days of moving into housing

  5. H1K CCP Service Delivery Goals • Case management sessions – at least one per week • Housing Trust Fund project, at least one 60 minute session each week

Service Transactions • What is tracked • Outreach Programs • Recorded for each attempt to locate a new client • Case/Care Management • Recorded for each meeting with client or work on behalf of the client • EHC Staff, for each Case Mgmt. service, entry on ezLabor time card • Housing Search Assistance • Information & Referral • Use “Add Service” to record • Provider is “H1K CCP” program • Record Units of Measure (Minutes) for the total time spent on the effort including travel time and case notes time (Case/Care Management only)

Case Plans/ Case Notes • Create a Goal for: • General Case Notes / General Case Notes • Write notes so that in your absence another person can pick up where you left off. • Abbreviations should be commonly used/agreed on. • To add new case notes, click on or click on to view existing notes • By default, Case Notes are shared between H1K CCP providers (except DADS). • Click on to change Visibility Groups (security). Communication

  10. Households (Summary Dashlet) • If client is entering the program alone, do not create a Household. • For H1K CCP program to be considered a household, must include at least one minor child • If entering program as part of a household do one of two things: • Edit existing Household to match current household arrangement • Use to enter “Date Client Left Household” • Start New Household • “Client” should be Head of Household. • If both adults are clients, then both HoH

  11. ROI (Summary Dashlet) • Add ROI (Release of Information). • “End Date” is three years from date of signing. • Documentation is: “Signed Statement from Client.” • Attach a scanned copy of the signed ROI.

  12. Entry (Summary Dashlet) • Entry should be done as soon as intake paperwork and ROI is signed. • On Summary tab, Add Entry / Exit • Provider: Agency Name “H1K CCP” • Type: Basic • Entry Date: Date paperwork is signed • Save and Continue

  13. Entry: SCC Standard Intake • Key fields to update: • Phone • Housing Status – “Literally Homeless” • Total time on streets/shelter • Living Situation Last Night • Income & Benefits • How old is the information? New Few Months Year or More Several Years

  14. Entry: SCC Client Snapshot • Performance Outcomes • Treatment Providers • Barriers to Housing • Used to identify problems that are preventing clients from gaining permanent housing

  15. Case Managers (Summary Dashlet) • Defines “My Clients” • This can be done at anytime • From Summary tab, click Add Case Manager • Type: “Me” • If necessary, previous Case Managers and add End Date as appropriate

  16. SSOM (Self-Sufficiency Outcomes Matrix) • SSOM measures are to be done: • Initial, at program enrollment or shortly thereafter • Interim, quarterly (every three months) • Final, at program exit • Use Matrix tool to calculate measurement: • santaclara.bayareahmis.org Self-Sufficiency Matrix • Data quality report shows upcoming Matrices due

  20. Interim Review • Create an Interim Review when client moves into housing (or other significant milestones) • the Entry, Save & Continue • Click on Interims . Click on Add Interim Review. • Interim Review Type: “Interim Review” • Update information on the Assessment
