  2. My Illness • I’m Welsh – people tend to think that’s a long term condition! • Heart Failure • Didn’t know what was wrong with me • Wife left – I was always asleep and too boring! • Off sick from work • Isolated – reduce social contact • Only do what you can

  3. What happened • There is always a hero/heroine • One stop shop diagnosis and management plan • Approved for TH • Next day installation and training • Pulse, weight, exercise – through my TV • Insiprational video and questions

  4. What Changed • I felt supported to manage my health • Confident to do things I though I could not • Informed about what was happening to me • No hospitalisation • Independence

  5. Background • I was a Director of Commissioning of a PCT • I commissioned the TH system • I was the first to use the TH system I commissioned • I now work for the Advanced Digital Institute and for Myself
