  2. Application for Project Proposals Initials on every page

  3. Application for Project Proposals Signed in blue ink and signature must be original NOT scanned Signature of Permanent Secretary in case of Government Entities

  4. Soft copy of Project Application Applications must be submitted in Word and Excel formats 5 Spiral-bound copies have to be submitted

  5. Log Frame of Project Implementation All project activities must be listed here

  6. Financial Identification Form and Bank Details Must be signed by the Bank Representative

  7. Declaration by Partner Organisation Partner organisation is only the one which will be providing a co-financing rate

  11. Example 1 – Staff Costs Assignment of a projects officer to be paid an hourly rate of 10€ for 50hrs Recruitment of a projects manager to be paid a monthly salary of €1500 for 1 year.

  13. Example 2 – Travel Costs 4 Delegates will be coming to Malta on 1st March and will leave 5th March having a per diem rate of 205€ The flight of each delegate costs €300

  15. Example 3: Sub- Contracting & Equipment Procurement of a venue for a total cost of €1,000 excl. VAT. Cost includes food items for the value of €500 excl. VAT. Cost also includes the cost of a projector for the value of €100 excl. VAT Procurement of a laptop for the use of the project for a total cost of €800 incl. VAT.

Proof of Co-Financing Audited accounts for 2012 must be submitted In case these are not available, the organisation should submit: • Audited accounts for 2011 • Management accounts for 2012 Required at the application stage in case of: • Voluntary Organisations • Non Governmental Organisations • Private Entities Also applicable for partners who are co-financing project and are not government bodies Not required in case of government bodies

Proof of Co-Financing Joint declaration by: Executive/Head & Treasurer/Financial Controller stating that they have sufficient funds to cover the co-financing share Required at the application stage in case of: • Voluntary Organisations • Non Governmental Organisations • Private Entities Also applicable for partners who are co-financing project and are not government bodies

  24. Certificate of Enrolment/Application/Exemption Applicants must submit one of the following: • Copy of enrolment certificate as a voluntary organisation • OR a copy of application and acknowledgement • OR Exemption from the relevant authorities Required at the application stage in case of: • Voluntary Organisations • Non Governmental Organisations • Private Entities Also applicable for partners Not required in case of government bodies

Europass CV of Project Leader and Organigram Europass CV of Project Leader required at application stage CV of legal representative is required in case: • Project Leader had not been recruited • Project Leader will be appointed at application stage Organigram/Organisation Chart of the Administration Structure of the proposed project required at application stage
