  1. Per the Collective Bargaining Agreement, as of the March 1, 2012 application deadline, there were 3 new requirements required for the application/renewal process: Reminder!!!! Required use of new applications and renewals, available on the nursing pathways website* for the application or renewal of the following designations: SN III/HH or H III SN IV NP III SN III/HH H III application now includes an expanded exemplar section for reference Use of new applications is required as of the March 1, 2012 application deadline 2. Signature of a Mentor is required on all clinical ladder applications and/or renewal packets, effective March 1, 2012 3.
